Submit Documents Electronically

To protect patient privacy, we ask that our patients not use regular email to send us documents or images. However, we do have a portal that can be used to send information securely.

If you wish to use this option to submit new patient paperwork, a copy of your insurance card, or other information, you can use the link below to do so. However, we ask that you call our office and notify us beforehand that you will be doing so. Please let us know what documents you will be sending and when you plan to send them. You will then click on the link below. It will redirect you to a pages that will ask for your name and email address. You will then receive an email containing a link and password that will allow you to access the portal and upload the documents/images that you are sending to us.
